Specific numeric ratings for each question, with comments


I am constructing a performance survey for a client who wants respondents to give a specific rating for each item, in tenths from 1 – 4. Example ratings could be anything on that scale. e.g.,  1.1, 2.3, 3.5, 3.8, etc. 

The report would average these values to generate a final score. 

I tried creating a slider and can do so in tenths, however, respondents cannot tell what the increments represent as they don’t seem to be able to be labeled. 

Can you suggest the best way for me to accomplish this?

    Have you considered adding a left, center, and right label for your slider? This could help clue respondents in to the values.

    You could also add instructions to the question to add some clarity. As soon as a respondent clicks on the slider value they should see the value associated with that spot, like so:

    You could also consider using something like a likert scale or radio button grid where you provide labels for each point on the scale:
