How to create a page like that? Where respondents can download a pdf after answering the required questions.

I am assuming that you want to send your respondents a pdf copy of their responses. This is possible – you can review the SurveyGizmo documentation for details – https://help.surveygizmo.com/help/send-a-survey-respondent-a-copy-of-their-response

Yes – this can be done two different ways. Your first step for either of the would be to upload the document to the file library – https://help.surveygizmo.com/help/libraries. Once the file has been uploaded be sure to copy its URL.
The first option is to create an email action on the THANK YOU page. Unfortunately you can’t attach the document to this email, but you can include a link to the document in the email body – https://help.surveygizmo.com/help/create-a-hyperlink. Your respondent can then use the link to view and download the document. This option requires that you obtain the email address from your respondent.
The other option is to place a URL REDIRECT on the THANK YOU page. This redirect would redirect the respondent’s browser to the uploaded document – https://help.surveygizmo.com/help/redirect-to-a-website
There are advantages and disadvantages to each option. You should test them to see which one will work best for you
You can set up a Review Action (https://help.surveygizmo.com/help/review-response-action) that allows respondents to review their responses before hitting the SUBMIT button. This action also allows respondents to download a copy of their responses in PDF format.
It is not advisable for your respondents to leave the survey session and view the pdf. This would increase the chances that your respondents would not return to the survey to hit the SUBMIT button and complete their response.
Rather than sending them via email, I would like them to see the pdf copy after clicking the download button. Is this possible?
