I have sent a survey to all my customers via surveygizmo…It’s already difficult to have answers but I have someone who is willing to answer but he’s not able to due to the fact he can’t tick the possible answers on the second question which is mandatory to continue.
Other people can complete the survey so I’m not sure it’s coming from you but I would like to try because his answers are very important to me.
I have attached what he has on his laptop and what he should normaly have.

Unfortunately, it doesn’t look like your attachment came through… The only thing I can thing of that this respondent may be running into is using Internet Explorer with high security settings. If that is the case, the advice here should help:
