        Use a Rating (Likert Scale) type question.  This question comes with the options pre-populated with a standard satisfaction scale (Very Dissatisfied, Dissatisfied, etc.).  You can edit options 1, 3, and 5 to match your existing question.

        You will not be able to erase options 2 and 4.  I recommend that you rename them something consistent with your existing scale and then use the HTML editor for that choice to change the font color to the same as the background color.  It would look something like this – <span style=”color:#FFFFFF;”>Dissatisfied</span>.

        This makes the options for 2 and 4 invisible to the respondents, but they are still there serving as placeholders for those options.  And they will also be available on your response reports.
