How do I make a fillable form for a question?

I want users to list supports they received and then rank each support on a scale of helpful to hindrance.  How do I set this up?  I imagine a table with blank textboxes in the left column, and then the scale in the next 3-4 right columns.

    Hi Kate,

    Typically two-part questions like these are asked with 2 questions. In the first you ask the type of support they’ve received. Then in a follow up question, you can pull in the options they selected and ask them to rank them. You can do this using something called piping. Here is a tutorial with more info:

    I should mention that we do not allow piping from an open-text field like you describe. This is because it can create very unwieldy data files. So, your first question would need to be a checkbox question where you provide the possible options.

      • Thank you, very helpful! One more question – if one of their options is “other” with a write in, will it still work? Would “other” show up in the report, but I could go back and see what was written in?

        Yes the “Other” option will pipe automatically. You can exclude it if you wish (this is covered in the FAQ of the tutorial I shared). But yes, to answer your question it will be pulled through. In the data, you’ll be able to review at the open text entries to the other option in the first question!
