We ask for several file uploads in one of our surveys, but in the PDF Review that’s emailed to administrators, those three questions are replaced with a small empty gray box.
Sorry about the troubles you ran into with this. Displaying images uploaded to the File Upload question within the Send Email PDF is currently not working due to some changes in how we store these images. This has been reported to our development team to be addressed but we don’t have a timeframe on when it will be resolved at the moment because of the trickiness of fixing it.
Our apologies, the best way to access those files at the moment is via the Individual Responses or the Bulk File Export.
