I’ve found help documents for including custom fields from an Email List in reports, but is there a way to include custom fields from a Contact List in reports or is it only included in downloads?

You could just create hidden values in the survey and set them to collect those custom fields from the contact list. Then when you create the report, make sure you include the open text data. Hope that helps.
